Refresh should always work.
It seems like it stops working whenever I'm in the empty page (i.e. showing 'Folder "Latest" has no unread items.'). If I press 'r' nothing happens but doing a browser reload brings up the new items.

No. I didn't released it. Probably you've added subscription and it updates comments counts and other unread counts (but unread count updating is a bit buggy)

There is no auto refresh at the moment. Initially I did that so new items don't disturb from reading. But planning to implement auto refresh in the near future.
'r' does not refresh it's more like test key and not even listed in keyboard shortcuts help. But I will change it.
